How do you add a Lookup Field in Helpdesk 6.0 SP5?

Answer
Adding a Lookup Field is one of the few customizations outlined in the Helpdesk Product Guide. Unfortunately, when the SP5 Product Guide was published, the steps for doing so were not updated to reflect the new code.

On the right-hand side of this article is a file you can download with accurate steps for adding a Lookup Field.
