Helpdesk cannot create new incident

Users going through the My Helpdesk console (Winuser console) are not able to create new incidents.
Notification Server 6.0.6074
Upgraded to Support Pack 3 for Helpdesk.
Add the domain users group to Altiris Guest and give the Altiris Guest group permissions to create incidents.
