Discovery error when configuring VOM to interface to Active Directory or LDAP domain for non root user authentication

Problem
BACKGROUND
VOM can be interfaced to an Active Directory or LDAP server for non-root administration login. See the following section in the VOM 4.0 Administration Guide.

Example to reach the LDAP/AD discovery wizard/template:
Select Settings / Authentication Brokers then click Configure LDAP/AD button.

Customer completes LDAP/AD discovery wizard as follows:

While this issue was discovered in the specified environment, it will likely be relevant to other versions and platforms as well.
Cause
Invalid syntax (missing input elements).
Solution
Corrected syntax format (as follows):
Bind username:
amer\SRVGBL-vomadmin
Query information (search base):
SRVGBL-vomadmin
