Solution

There's two things that could be causing the issue (note - the server names are examples):

Case 1:

- vxsvc is hung/not behaving correctly:

1. You must verify vxsvc status
# root@<server1>:/> /opt/VRTS/bin/vxsvcctrl status

Current state of server : RUNNING

2. If the service is running but not showing VxVM objects, restart it:
# root@<server1>:/> /opt/VRTS/bin/vxsvcctrl stop
Stopping Veritas Enterprise Administrator Service (vxsvc)
VEA Server was shutdown successfully
# root@<server1>:/> /opt/VRTS/bin/vxsvcctrl start
Starting Veritas Enterprise Administrator Service (vxsvc)
...VEA Server has successfully started
3. Now restart VEA and verify there's no errors:

Case 2:

- Storage Foundation is not configured correctly:

1. Check service status:
root@<server2>:/> /opt/VRTS/bin/vxsvcctrl status
Current state of server : RUNNING

2. Restart the service and watch for errors:
root@<server2>:/> /opt/VRTS/bin/vxsvcctrl stop
Stopping Veritas Enterprise Administrator Service (vxsvc)
VEA Server was shutdown successfully
# root@<server2>:/> /opt/VRTS/bin/vxsvcctrl start
The server is not configured. Unable to start the server.
Please configure before running.

3. Run installsf -configure:
# root@<server2>:/mnt> /opt/VRTS/install/installsf -configure

Choose how this install will be managed.

This product can configured one of the two following ways:

      * Storage Foundation Management Server managed host (Recommended)

      * Standalone host

Pre-requisites for Storage Foundation Management Server managed host:
              * A Storage Foundation Management Server (SFMS) is configured
              * SFMS should be currently running and fully operational.
              * Authentication Broker that is used by SFMS should be running.

The following information would be requested during configuration:
              * The host name of the SFMS
              * Previously set SFMS agent account password

There are no pre-requisites for deploying as a standalone host.

Enabling Storage Foundation Management Server management simplifies and improves management of the complex data center resources, reducing planned and unplanned down time.

Enable Storage Foundation Management Server Management? [y,n,q] (y) no

NOTE: <--- Select YES if you need to configure VxSS Otherwise, if you do not have an FQHN configured, do not choose to configure the SFMS (Storage Foundation Management Server).  Answer "no" to the configuration of SFMS. The default answer is "yes" to this question.  So it is very easy to answer "yes" to it accidentally. Answering "no" to the Storage Foundation Management Server question will allow the installation and configuration to complete successfully.
