Disk show as Failing or with a FAILED status in the Veritas Enterprise Administrator

When opening the Veritas Enterprise Administrator (VEA), in certain situations, disks may appear with a Yellow Caution symbol on them, and list their state as 'FAILED', with the disk and volume offline.
Storage Foundation will list a disk as FAILED with a Yellow Caution Symbol if the disk does not respond to an I/O query sent by SFW.
In this instance, the disk is 'flagged' with the caution symbol as being a potential problem that needs attention. This does not necessarily mean that there is a hardware issue with the disk(s), but instead suggests the disk was flagged for failing to respond. In circumstances where the disk(s) may actually have a hardware issue, the SAN team (or team responsible for maintaining the SAN) will need to be engaged to run diagnostics on the disks themselves, or the hardware that is used in conjunction with the problem disk(s).
To remove the FAILED status flag from any disk, simply perform these steps:
- Right click on the disk in a FAILED state
- Select the option to Re-activate Disk (click YES on any messages that may come up)
This should bring the disk back into an active state if it had indeed been flagged for a failed I/O request.
If there is an actual issue present with the disk(s), then shortly after Re-activating the disk, it will go back into a FAILED status. If this occurrs, please contact the SAN team for troubleshooting the problem disk(s).
