Fusion-IO disks not claimed or disappear from configuration after update Fusion-IO driver 3.x or later

Problem
After updating the Fusion-IO driver to 3.x or later from 2.x versions, the Fusion-IO disk devices are no longer claimed by the DMP Array Support Library (ASL). The devices are visible and accessible with operating system commands and may appear in /etc/vx/ddlconfig.info but are not available in DMP or Volume Manager.
Error
Fusion-IO disk devices are present in OS device tree and accessible but fail to be claimed by DMP.
Environment
Storage Foundation / Storage Foundation HA 5.1SP1 for Linux.
Cause
Fusion-IO has changed the output of 'fio-status -v' in driver version 3.0 and later.
Solution
Update to Storage Foundation 6.0.1 or later where the ASL has been updated to address the changes.
A fixed ASL for 5.1SP1 is not yet available however has been requested.
