Advanced Ticket Form in Service Desk 7

Hello,
I am testing the SD7 solution and I want to open an Advanced Ticket Form. I tested it with the Admin and with the Technician, but it doesn't work: a blank form is displayed instead of the form and stays until I close it.
Has anyone encountered this issue?

Depending on your hardware,
Depending on your hardware, it can take a little time before it opens. I am testing on a VM with 4GB and the form shows blank 1st but after a minute it shows up.
