  • 1.  All New Incident forms are either blank or they don't come up

    I have a relatively fresh ServiceDesk install.  After installation, when I click submit Request, then click Create Incident, Close Incident or Create Incident Advanced, a window opens but its blank.  I tried to fix by editing the SD.Feeder.GeneralIncidentSubmitForm thinking that's what I needed to do and now when I go to Submit Request there is a SD.Feeder.GeneralIncidentSubmitForm button above the Create Incident Advanced button.  How do I get rid of that and get my forms to open.  The other forms such as Sumbit a Problem or Request Change work fine.

    If you are seeing SD.Feeder.GeneralIncidentSubmitForm button in the forms list, it looks like a duplicate form has been published to the portal in the ServceDesk category of the Service Catalog.

    Go to Admin > Service Catalog Settings > ServiceDesk, see SD.Feeder.GeneralIncidentSubmitForm listed in there, click the lightning bolt icon and choose Delete.

    Second, when the form hangs blank when you try to open it, go to All Programs > Symantec > Workflow Designer > Tools > Log Viewer and see if there are any timeout errors or any other related errors thrown by the process. This would give an idea to what could be the cause.
