backup fails after restore?

Running BackupExec 9.1 rev 4691.
 
We have been having an issue where our backup will intermittently hang at 317,646 bytes.  When we check the log in the AM, the backup shows still running but is stopped at this byte count.  We can cancel the job, and we usually reboot the server.
 
At the end of the log, it says:
 
Backup - C:
The media operation was terminated by the user.
Error reading file data.
Error reading file data.
 
One thing we've noticed is that that this seems to be happening the day after we run a restore.  If it were once or twice I'd consider it a coincidence, but looking back at the logs it has happened EVERY TIME we've done a restore, 7 or 8 times in the past 3 months.
 
Any ideas on how this could be related?

Known bug with a HotFix that was rolled into SP1
 
Apply SP5 and you should be good to go

Can you tell me how to tell what version of the service pack I have installed?

I went ahead and installed SP5, but in HELP, ABOUT it still shows the same rev.
 
Is there some other way to confirm that SP5 installed successfully?

Only way that I know of is to check time stamps on some of the files in the Service Pack
