BE 12.5 to BE 2012 Upgradation

Hi Team,
Currently we are using BE 12.5 on Windows Server 2003. Now we are going to upgrade BE 2012. Kindly provide the what are the pre-requirement for this activity and how we will make this activity successfull.

You might have to do a bit of reading. Will recommend to perform a mock upgrade on another system and then try on production
