Best backup procedure forms

Dear all,
I am looking for a document, ready to be filled by each Department Administrator, about the backup procedures of each department's data.
I mean, what to be backed up, how often, retention policy, what is the acceptable restore point etc
Is there something ready from someone else?

You should really define a company standard instead. The strategy should be based on input from stakeholders and written requirements along with a risk assessment. The risk assessment should be reviewed/commented by at least one with VP status. Do not forget to put a cost of covering each requirements
Do not make policy/schedules and retention a candy store. You may end up tweaking and stretching the backup system to do things it was never intended to do.
Assumption is the mother of all mess ups.

I totaly agree with Nicolai. If you went with what the different depts wanted...you would have a mess.
