Data: HP DL360g5, Windows Server 2008 x64, Veritas Storage Foundation 5.1, Failover cluster, HP StorageWork MSA 2012fc, HP 4/16 San Switch.
When I configure dynamic groups in SFW 5.1 the server gives an error:
STOP: 0x00000007e (0xffffffffc0000005, 0fffffa6000d489bd, 0xfffffa600ad78738.......
msdsm.sys Adress fffffa60000d489bd base at fffffa6000d3d000
P.S.
Without HP 4/16 San Switch, everything works perfectly !!!!

There are no unsupported or specific switch configuration exceptions for Storage Foundation and High Availability Solutions 5.1 for Windows.

1. Problem resolved by installing patch:

2. Wiring made on your link, I had only one zone. Connecting specified in the link, I think better.
