Helpdesk 6.x incident request form with workflow 6.5

just seeing if any has already build a incident request form for Helpdesk 6.5 using workflow. We are going to put one in place temp until we can get SD7 setup.
Anyone already have a .Package of a request form for it that you have built?

I'm working on the same thing. I got a basic form build but I want to be able to link to the KB as well as show the user their created tickets. How far have you gotten on this?

I've got a package
I have a simple package put together that presents the user with a form to fill out then creates a helpdesk6 ticket. This was actually my first project. I don't have anywhere to host it right now but if you send me a PM with your email address, I'd be happy to send it to you...assuming you still need it. You'll either have to change a few things to fit your environment or just use it as a guide to create your own but either way, it shouldn't be that hard. If you have a way of hosting it, please be my guest as I'm sure others could benefit.

I didn't get that package but thanks for the offer, i wasn't able to wait to long and had one of my guys get one together. We also got the file upload to work to the old helpdesk. I'm guessing that you had to use a blank simple text box for the entry page ? we tried the advance first and the old hellpdesk can't handle it very much .
