Media Mount Fail after backup. Veritas Backup Exec 10d Rev: 5629

My Full Backup is failing with a Media Mount Failed message at the Verify stage.
Data is being backed up and I can restore from the tape.
The backup runs but at some point the tape is ejected and tells me that it is Overwrite Protected or words to that effect.
The tape itself is not in the 'lock' position and the software is set to overwrite and reset the archive bit.
No matter what tapes I insert they are rejected with the same error and I have to cancel the backup resulting in the Media Mount Failed notification.

Verify

Media mount failed.

User canceled a Physical Volume Library operation.

I have checked the settings which as far as I know have not been changed and the backup worked perfectly until recently.
The Daily backup (incremental) works perfectly.
The tapes have a 400GB capacity and the Full backup is about 165GB in size.

Any suggestions?

Before the verify fails, it

Before the verify fails, it must give you message specifying the label lf of tape that it is requesting. Make sure to insert the correct tape at that point to avoid this failure.

Having said that the tape we use has been used regularly for the same backup on the same day for many months without this issue.
Why would it suddenly demand a specific (the same) tape?
