If the SEPM server keeps the same IP and host name, you can refer to "Best Practices for Disaster Recovery with the Symantec Endpoint Protection Manager"

But as you stated it would have different hostname & IP address then you will have to perform following steps.
1) Install a new SEPM
2) Restore the database so that you would get policies and group structure but communcation won't start.
