I am running SEPM MR1.  It is installed on C but it's low on space.  Can I backup the db, reinstall MR4 MP2 and restore the db?  Or do I have to upgrade the db first?

Also, can I upgrade directly from MR1 to MR4 MP2?

If you have less number of configurations and clients..
I will suggest remove MR1 SEPM..then install MR4MP2 SEPM on D drive( or any drive or any other computer you want )
Run sylinkreplacer to re-connect all the clients to new SEPM 

But if you want to upgrade and do everything at once then the procedure is diffrent
First upgrade to MR4 --it should be on C
Then Upgrade to MR4MP2 --it should be on C

Once the SEPM is on MR4MP2
then follow the Disaster recovery process and change it to any other drive you want.

The reason why you should not migrate SEPM from MR1 to MR4MP2 directly is because the database schema for both the versions is different and doing so might corrupt the SEPM database. So the best practice is to migrate to MR4 first and then to MR4MP2.

If you want to change the Drive then its better to do an uninstall & reinstall & then use the sylink replacer as Vikram said.

Fresh Installation will good.
