the scheduled reports of my SEPM shows some of my clients are still running on OLD DEF of SEP but when i check the entire system, i cant find those systems. i even deleted all the systems from SEPM and run the reports after couple of days, the problem persists
any clues?

are you using image os ?
Are SEP client showing online in SEPM console ?

the OS image is not GHOSTED / i cant find the systems with those outdated virus def as reported by the SEPM scheduled report, but when ever i run the report or schedule the report, the message creeps in
Iam also having the same issues..scheduled report is showing old date but clients are up to date only?
Could you check if the Disk space on the SEPM server?
Try Increasing the heartbeat interval to feasible value (eg. 500 computers 1 hr interval).
Delete entries from data/inbox/agentinfo folder and .tmp files from data/inbox.
Hard disk is having more than 150 GB free space
