The forms that you posted are perfect!   You wouldn't happen to also have a copy of the APRF that you could share would you?

What you need is a requirements specification document. It can also be called as "Application Package Intake Form" or simply "Package Intake Form".

After getting the package details through Package Intake Form, you need to evaluate the package to check whether it is a legacy application or a windows installer based application. Then go ahead with Setup Capture or TRANSFORM creation depending on the result. The process is called "Technical Review/Assesment" or simply Tech Review.
