Pre-application packaging information to gather

This issue has been solved. See solution.

Does anyone have a canned form that they use to gather all of the information they need to create a package?   I.E.   Business Unit Contact, media source, where installation files are located, special configuration notes, etc.? 

We are just beginning to package over 3000 applications and we're going to use people in the field to help gather information, just looking for a standard format to hand them.   I can create one but thought it was better not to reinvent the wheel if there is one that exists.

Application Packaging Intake Form

What you need is a requirements specification document. It can also be called as "Application Package Intake Form" or simply "Package Intake Form".

After getting the package details through Package Intake Form, you need to evaluate the package to check whether it is a legacy application or a windows installer based application. Then go ahead with Setup Capture or TRANSFORM creation depending on the result. The process is called "Technical Review/Assesment" or simply Tech Review.

I have published some documents which will help you in your process. Please take a look at them and modify them according to your requirement.
