what should i do if my migration and upgrade was not successful?
any suggestion for rollback plan?...

Are you taking about the SEPM
Are you taking about the SEPM or the SEP
If the SEPM upgrade rolls back, you would still be able to login to the SEPM of the prevoiys version.

Yes , if you have a backup
Yes , if you have a backup taken of the previuos version.
Say you are on RU6 and want to go back to RU5
For SEPM:
Uninstall RU6
Install RU5 and restore the backup that you have for RU5
For Clients
The only way is to uninstall RU6 and Install RU5
