SEP 11 Manager not getting all signature updates from the Clients

My SEP manager 11.4000.2295 seems to update all of the clients correctly, but when I review them on the main page or individually in the CLIENTS tab, the signature date shows an older sig file. If I perform an update on the client, still no change in the manager.

I checked the clients area, had to modify the DEFAULT since they are or should be all inherited. Under the OVERVIEW, some groups are highlighted and others are greyed out. I'm not sure that this is the problem, but it does look like a problem. As far as I can remember, I didn't or wouldn't change any settings here for livestate, but it does appear inconsistent.
For me when I look at a group of 20+ clients on a page, only one or two will have a previous date displayed. The client has a green dot on it and when I have RDP'd to the client and look at the version of SIG it is up to date.

Create separate groups and assign the polices to them by right click, atleast you know what policy settings you applied for the clients.

Ok, for two groups I turned off inheritance. For one client in one group, the SEP mgr shows an outdated SIG, but when I log into the computer, the AV is updated, but the proactive threat protection is from Febuary. The other group with an outdated Sig displayed in the manager has both the AV and Proactive upt to date.
I didn't change any configuration in the LIVEUPDATE for either group, I believe they are all defaulted. We only have installed AV and Proactive modules at the moment. No one else has access to this manager but me.

I'm scared to upgrade
Based on some of the stories I have read and...
The last time I had MR3, I could never get the clients to upgrade and communicate. I would install over the top of the existing version with mr4, and it would never communicate back to the Manager. I had to uninstall, reboot and reinstall, then I was always successful in getting the workstations to communicate...until now. It looks like I have about a 5% failure rate eventhough it is properly distributing the signature updates.
