Service Desk Install is not creating all the database tables

Last week, we had an issue with our Notification Server that required it to be reinstalled (the CMDB was intact). Since then, we have had some major problems with our Service Desk and have tried to reinstall it. It was nearing its go-live date, so no data was lost.

I have tried to reinstall the Service Desk/Workflow to see if that would correct the issues we were having. Now the issue is that the install does not create all the required tables in the database.

The tables that are missing are:

dbo.CmAssignment

dbo.CmCabVote

dbo.CmChangeApprovalBoard

dbo.CmChangePlanTemplate

dbo.CmChangeTicket

dbo.CmRelatedProcess

dbo.ServiceDeskBlackList

dbo.ServiceDeskCategoryMapping

dbo.ServiceDeskChangeManagement

dbo.ServiceDeskIncidentManagement

dbo.ServiceDeskOverAllSLSchedules

dbo.ServiceDeskProblemmanagement

dbo.ServiceDeskProcessEscalationHistory

dbo.ServiceDeskRoutingRules

dbo.ServiceDeskSLA

dbo.ServiceDeskSLALevelmanagement

dbo.ServiceDeskSurveyData

Any help would be greatly appreciated.

Used the test database to get the SQL code to create the missing tables.  Once created, the rest of the install proceeded normally.
