If you open the SD.Feeder.TechnicianIncidentForms project and double click on the Create Incident Form, you will see the Extend Classification links above the Type dropdown list. Double click one of them, and on the window that pops up, you'll see a User Interaction tab. Click the ... next to the forms model, and you'll have access to the Extended Classification form. Bear in mind that a lot of this data is auto-populated from the CMDB (if you're using it) from the "Fetch CMDB Data for Dropdowns" embedded model. You can edit the dynamic models in each of the dropdowns if you want.
