Can anybody tell me a) what causes a device to become blacklisted in app centre and b)how to remove a device from the blacklist ?

Hello,
A device becomes blacklisted if a "Revoke" command is issued for the associated user of that device. The blacklist can only be removed by executing a "De-provision" command against the device.
