I also get this email:
A password reset initialization attempt was started by from machine name TSTSD701 and IP address 10.100.8.34.
During the "Get Current User AD info including Security Q&A" component execution, an error occurred.
This is most likely an error reaching the Active Directory Server, either due to connectivity issues, invalid AD password, etc. Please investigate the issue and contact the requesting user to let him or her know to retry the password reset initialization. The exact error message was:
An item with the same key has already been added.
