This is happening to an active account and apparently to a specific document.  Have searched the user's mail file for it and deleted all instances I've come acrossed.

I think you already tried this:

If you still have the same issue then the mailbox has some corruption. YOu can try

Also you can try rebooting the server or dismounting and mounting the store to resolve this.

I faced the same issue on couple of XLS files and asked the user to delete the email containing the documents...

Its a best practice to have a "Support Contract" with Symantec...
