Incremental backups failing with error after extending vmdk
Just experienced the same error on my Proxy server for a specific VM.
All other VM's performed Full and Incremental backups.
This VM successfully performed a Full but gave the "Cannot open disk blklst" and "Cannot open disk file: Error : Disk library failed" error message when performing a Incremental.
I did a restart of the VMware Vmount Service and now successful Incremental backups.
This specific VM started to fail Incremental file-level backups just after having a vmdk extended. This might have something to do with it.
This technote on NetBackup also helped me.
All Incremental VMware Consolidated Backups (VCB) are failing with error 156 and bpfis reports the error "Cannot open disk file: Error : Disk library failed (Unspecified error)"

Tip: Get overview/document your NBU environment. Run 'nbsu' and review the output.
