Version of SF/VxVM?
Platform appears to AIX from the command output - version?
Specific/relevant messages reported by the vxdmp driver might also be helpful??
Note: if this is the same server/configuration as your other post [SF 4.0MP4 on AIX 7.1] ( https://www-secure.symantec.com/connect/forums/vxd... ) then as Marianne has pointed out, this is not supported, so you need to upgrade to a supported version/platform combination.
