Where are the change managment risk assessment, scheduling, etc, forms?

I'm using ServiceDesk 7.0 MR4. I want to make a change to one of the change management forms - the 'Risk Assessment' form. However, I haven't been able to find the form in the workflows. The SD.Feeder.ChangeManagment.WebForms project ends prior to getting to that point so the form doesn't exist there. I looked in the SD.ChangeManagment.Workflow project and I see the steps/models for Risk Assessment, Scheduling and Plan Creation, but there are no forms associated with those that I can find. I'm sure I'm just missing something simple. Where can I find those forms?

All the forms you are looking for are going to be found in the SD.ChangeManagement.workflow project. You will need to go into each of the specific linked models for Risk Assessment, Scheduling, & Plan Creation. 

Once inside each model you will find a dialog workflow component. For the Risk Assessment model the component name is called 'Risk and Impact Assesement'. Open this component and go to the Interaction Setup tab and click on the ellipses for the dialog model field. Once you've done this you will find the actual form.
