Can someone explain exactly what  Windows System Assessment Scan does?

It scans the machine to determine the applicable updates that were imported from the windows patch data import(ms, adobe, Mozilla, etc). This is what tells you whether the machine has applicable patches within the remediation center or not. Does that help you?

And why it is always in 'Deliveries available for this computer' on a client?

The reason is that new version of PMImport will update vulnerability assessment package on clients.
