There was nothing with the workflow config as I suspected.  This was an issue in the SQL server set up and once the SQL admin was able to set up the connection string correctly, I was able to connect and everything works fine.

I hadn't thought of that.  I think I might take the HTML from the final webform and see if I can add that to the HTML convert instead of the HTML builder.  It's all about aesthetics at this point.  Though I could care less, ...

I have found my problem.  The domain name must be fully qualified not just the netbios name and must not include the server name.
