    Please check these steps below from https://vox.veritas.com/t5/Desktop-and-Laptop/error-message-information-v-138-32772-3604-failed-to-load/td-p/819691

    1. First follow this TechNote http://www.symantec.com/docs/TECH60627 to make sure that all the settings on the client workstation is properly configured.
    2. If the step 1 doesn't fix the issue, run MDAC (Microsoft Data Access Components ) Component Checker to check for any mismatches.
    3. If the mismatches are found run repair of MDAC.

    Thanks for your reply.

    I checked the registry settings from the article that you supplied and there's no DLO entry on the client system and everything looks ok on the Media Server.

    How do you run MDAC?
