I have seen such issues before, delete that notification and create a new one..

    what version you are on?

Multiple Symantec Endpoint Protection Manager email notifications are sent for old events

Something is "stuck" in the database. Delete the alert to clear it out and re-create it

Also, what's the exact SEPM version?

    Version 12.1.4013.4013

Is it possible the machine was offline for several months? Most notifications are normal and it works good.

It could be but you'd have to check :)

Can you export it to a .cvs file? Then it will show you the event time and the event insert time. The difference is the "Event Time" is when the event happened, the "Event Insert Time" is when the event was logged in the Database. If the device was unable to communicate with the database this would delay the notification of the event.

you wont get notification for offline clients coz notificiations rely on data sent from client like 24 hours, etc.... just delete the notification and create a new one.

Yes maybe thats the best idea to delete and create a new one.
If this behaviour will return in the future i will inform you guys.

    Thanks,
